Every vehicle registered in Massachusetts needs a safety inspection once a year at a licensed station, and the state fee is $35 for most passenger vehicles. Newer gasoline cars, trucks and SUVs get an emissions check through the on-board computer during the same visit. If the car passes, the inspector puts a new windshield sticker on it with a number showing the month it expires. If it does not pass, you get a rejection sticker, a printed report and 60 days to fix the problem and retest.
What the safety inspection covers
The state's Mass Vehicle Check program evaluates 14 groups of components, and the safety portion takes roughly 12 minutes once your car is on the lift or in the bay. The list is shorter than many drivers assume, which means most failures come from ordinary wear rather than anything exotic.
| Area checked | What the inspector looks at | Something you can check first |
|---|---|---|
| Registration, plates and VIN | Valid registration, plates in good shape, VIN matching the dashboard | Plates not bent, cracked or unreadable |
| Brakes | Service brake and parking brake | Pedal feel, noise, pull to one side |
| Exhaust | Muffler and components, excessive blue or black smoke | Loud rumble or visible smoke at startup |
| Steering and suspension | Steering wheel and box, front end, springs, shocks | Clunks over bumps, loose steering |
| Horn | Works and is mounted securely | Press it once |
| Glass and wipers | Windshield, windows, wipers and washer, window tint | Cracks in the driver's line of sight, worn blades |
| Mirrors | Rear view and side mirrors | Loose or broken glass |
| Lighting | Tail, turn, hazard, reverse and plate lights, headlight aim, aftermarket lights | Walk around the car with a helper |
| Tires and wheels | Tires and wheels | Tread wear, bulges, pressure |
| Body and frame | Bumpers, fenders, floor pans, fuel tank, frame or unibody | Rust-through or hanging panels |
| Ride height, seat belts, airbags, fuel cap | Altered height, belts working, airbag warning, cap present | Dashboard airbag light, missing gas cap |
Rust matters here more than in warmer states. The floor pans, frame or unibody and fuel tank are all on the list, so an older car that has spent years in winter salt can fail for structural corrosion. The article on protecting a car from road salt explains what to watch.
The emissions test
For eligible vehicles the emissions test is an on-board diagnostics (OBD) test. The inspector connects an analyzer to the diagnostic port under the dashboard, and it takes about three minutes. The analyzer asks the car's computer three things: can it communicate, has it finished its own self-checks (readiness), and does it show a check engine light or stored trouble codes.
As of this writing, the OBD test applies to model year 2012 and newer passenger cars, trucks and SUVs, along with some newer diesels. The state describes the rule as covering vehicles 14 model years old and newer, so the cutoff year moves forward over time. Vehicles older than the cutoff skip the emissions test but still get the full safety inspection, and a car that smokes heavily will fail on safety grounds. Electric vehicles, motorcycles and new vehicles delivered to Massachusetts owners are exempt from the emissions portion.
Two points catch people out. First, a lit check engine light is a failure by itself. Second, a car can be rejected as "not ready" with nothing actually broken, usually because the battery was disconnected or codes were cleared recently. For model year 2012 and newer gasoline vehicles, one non-continuous monitor may be not ready; more than that fails. The state says about a week of mixed city and highway driving normally resets the system. Our guide to the check engine light goes through the common causes.
What to bring and what it costs
- A current registration document. The VIN on it must match the one at the left front of the dashboard.
- The $35 fee. Stations set their own payment methods, so call ahead if you want to pay by card.
- A clear path to the diagnostic port. If your car has a cover or clutter over it, clear it first.
Motorcycles are inspected separately at licensed motorcycle stations for $15. Commercial vehicles have their own inspection, and stations may charge more for it. The state also suggests going around midday on a weekday early in the month, since stations are busiest on weekends, early mornings, late afternoons and at the end of the month.
When the inspection is due
- Annually. Get it done before the sticker's month runs out.
- After a change of ownership. A vehicle must be inspected within seven days of being registered to the new owner. Buyers should read how to inspect a used car before buying, because the seven-day window also feeds into the state's lemon aid rule.
- If the car is out of state. Submit the Out-of-State Verification Inspection Form to avoid a registration suspension, then inspect within 15 days of coming home. If the other state has no inspection program, the window is 3 days.
Preparing so you pass the first time
- Replace burned bulbs and worn wiper blades yourself. They are the cheapest failures to prevent.
- If the check engine light is on, diagnose it first rather than hoping.
- After any repair that involved disconnecting the battery, drive normally for several days before you go.
- Have worn brakes looked at beforehand. The brake repair guide explains what a thorough check includes.
- Keep the registration in the glove box, not at home.

Frequently Asked Questions
How much does a Massachusetts car inspection cost?
The state fee is $35 for most passenger vehicles. Motorcycles pay $15, and stations that perform commercial inspections are allowed to charge more for those. A failed car gets one free retest at the same station within 60 days.
Does an older car need the emissions test?
Not if it was built before the current cutoff, which is model year 2012 as of this writing. It still gets the full safety inspection, and excessive smoke from the tailpipe is a safety failure. The cutoff moves forward, so check the Mass Vehicle Check site for the current year.
How long does an inspection take?
The safety portion takes about 12 minutes and the OBD emissions test about three. Waiting time at busy stations is what stretches the visit, so midday weekdays early in the month are the quietest.
What if my car is out of state when the sticker expires?
Submit the Out-of-State Verification Inspection Form with a copy of the passing inspection from that state. When you return, inspect within 15 days, or within 3 days if the other state does not inspect vehicles.
I just bought a used car. How soon must I inspect it?
Within seven days of registering it in your name. Doing it as early as possible also protects you if the car fails, because the state's lemon aid law has a seven-day inspection condition.
